How do I know if my child has a disability and needs more support?

What plan is my child eligible for?
Individualized Family Service Plans (IFSP) For Children under 3 years of age, there are early intervention services and plans. Talk to your health care provider or see here:

504 Plan For students in grades k-12, a child must have any type of disability that interferes with the ability to learn in a general education classroom. The disability must substantially limit one or more basic life activities.
Individualized Education Plan
For students 3-21 years old, a child must have one or more of the 13 disabilities listed in IDEA. These include: hearing impairment, visual impairment, autism, intellectual disability, communication impairment, emotional regulation impairment, multiple disabilities, deaf/blindness, brain injury, orthopedic impairment (condition that affects bones, muscles or tissues), other health impairment (includes multiple conditions, such as, ADHD, Tourette’s, epilepsy, diabetes, etc.), social maladjustment, specific learning disability, or development delay(s) for preschool. The disability must affect educational performance and/or the ability to learn and benefit from the general education curriculum. The child must need specialized instruction to make progress in school.

Once you have a plan, help your teachers get to know your student. Provide a Positive Student Profile (try to keep to 1-2 pages) and their list of accommodations and modifications.
